PharmGKB is the pharmacogenomics database that encompasses clinical information such as dosage guidelines, drugs, gene-drug associations with actionable clinical potential and relationships between genotype and phenotype. It is a publicly available tool developed by Stanford University with funding from the NIH.

It is a good source of information for clinical research into the effects of pharmacogenomics, both for drugs in development and for commercially available drugs.
